## About the Program

Our Medical Continuity Program is staffed by board-certified professionals, family medicine physicians and nurse practitioners. Our MCP program is available nationwide, Monday-Friday, year-round. Our providers hold single-state and multi-state licenses, and are credentialed to handle referrals from clients across the country.

### What to Expect

- Schedule availability for both initial intake and follow up visits
- AMG patients brought to you from our hospital, health system and health plan partners
- Focused visits for established chronic conditions
- Medication management
- General wellness and screening exams

### Your Qualifications

- Minimum 1 active state license
- Current board certification in family medicine
- Minimum 5-years post-residency experience for physicians OR 5 years of clinical experience for nurse practitioners
- Active, unrestricted DEA license
